The UCCS Clinical Site Sheet and how to get it right

The Clinical Site Sheet is the document that sets every UCCS graduate rotation in motion. You type it into that semester's Word version, name one site and one preceptor with an exact hour count, and send it to the Academic Services Professional for Graduate Nursing, whose office uses it to build the Affiliation Agreement and preceptor letter.

What the sheet sets in motion

Nothing about your rotation moves at UCCS until this form arrives. The Graduate Nursing Department drafts the Affiliation Agreement and the preceptor agreement letter from its details, and the Academic Services Professional who processes it decides whether the site also needs a myClinicalExchange rotation request. Questions about whether a site suits your course go to the Graduate Clinical Practicum Coordinator. The sheet is also the record UCCS relies on for whose hours count, so it matters well beyond the paperwork stage.

The name has changed over the years. The 2024 practicum handbooks called it the Clinical Site Information Sheet; the 08/2026 handbooks call it the Clinical Site Sheet. The RN to BSN program keeps a site sheet of its own for NURS 4450, a separate form with separate deadlines, so make sure you are reading guidance for the graduate version.

UCCS's rules for the sheet

The 08/2026 practicum handbooks spell out how the sheet must be prepared. Each rule below is a common reason a sheet gets returned:

  • It arrives by email to every clinical student at the start of each semester, and you must use that semester's version.
  • It must be typed and sent as a Word document attachment.
  • You need one sheet for each site and one for each preceptor.
  • It gives the exact number of hours. In the handbook's words, "Ranges of hours will not be approved."
  • A new sheet is required every semester, even when you stay at the same site with the same preceptor.
  • You fill it in yourself; the preceptor does not sign it.
  • Before your first sheet, UCCS asks you to review the Site Sheet Information Module in the Canvas course Graduate Practicum Resource(s).

What the sheet needs from you

The graduate form goes out by email and is not posted publicly, so its exact fields are not something this page can quote. UCCS's rules still make its core plain: a named site, a named preceptor and a firm hour figure for one semester, all checked against the credential and setting rules for your track.

The public site sheet UCCS uses for NURS 4450 hints at what a contract needs from a site. Its fourth section asks who holds "Legal Authority to Sign a Clinical Education Agreement" and warns that this is most likely not your mentor. The graduate process ends in the same kind of contract, so learn who signs for your site before you send anything.

Expect UCCS to check the preceptor against the credential rules for your track, set out in preceptor requirements. The Preceptor Handbook also asks preceptors for a resume where one is available, so it helps to have that ready.

Exact hours, and why a range comes back

The hour figure has to be a single number because UCCS builds the agreement and your rotation record around it. Your course sets the target: NURS 6915, for example, carries 110 hours for students who started in Fall 2026 or later and 90 for earlier starts. The clinical hours guide lists every course by cohort.

If two preceptors share a course, each gets a sheet, and the two figures should add up to what the course requires. Watch the running totals too: across the program, an FNP or AGNP student can count at most 330 hours from any one preceptor or site, unless that site sits within a major health system.

The sheet also protects your hours. Time with a preceptor who is not on a processed site sheet stops counting beyond 16 hours, so a preceptor you pick up mid-semester needs a sheet of their own without delay.

UCCS does not approve hour ranges. Write the one figure your course and cohort require, not "90 to 110."

Who receives it, and when

The sheet goes to the Academic Services Professional for Graduate Nursing. UCCS wants it well ahead of the rotation, ideally before the term that comes ahead of your practicum; the lead-time guide turns that request into dates for Spring and Summer 2027.

After it arrives, the same office emails you three times: when the agreement process opens, when it is complete, and when you may start hours. The stretch in between is the Affiliation Agreement and preceptor letter, covered in Affiliation Agreements. If a few weeks pass without that first email, a brief, polite check-in with the same office is reasonable. A sheet that stalled is far easier to fix early than on the first day of term.

Mistakes that send a sheet back

  • Reusing a saved copy from an earlier semester instead of the version UCCS emailed this term.
  • Writing a span of hours, or leaving the figure for the preceptor to settle later.
  • Putting two preceptors on one sheet.
  • Naming a relative, a close friend, or a site where you or a family member works; see workplace and family rules.
  • Choosing a setting your course does not count, such as inpatient rounding for FNP and AGNP students.
  • Sending a PDF or a photo in place of the typed Word file.
  • Leaving the site's contract signer unknown, which slows the agreement once drafting begins.

Each of these costs days at the front of a process that already runs on weeks. A clean first sheet saves more time than any follow-up email can.

Does my preceptor have to sign the Clinical Site Sheet?

No. You complete the sheet yourself, and UCCS's handbooks note that the preceptor does not sign it. Your preceptor signs a separate preceptor agreement letter, which UCCS drafts from the sheet, and someone with contracting authority at the agency signs the Affiliation Agreement. Share the sheet's details with your preceptor anyway, so the letter holds no surprises.

Do I need a new site sheet if I keep the same preceptor?

Yes. UCCS requires a new sheet every semester, even for the same site and preceptor, and it must be that semester's version of the form. Going back to a familiar site can still save time elsewhere, since an agreement already in place may spare you a new legal review.

Can I list a range of hours if my preceptor's schedule is uncertain?

No. UCCS approves only an exact hour figure, so settle the schedule with your preceptor before you send the sheet. If plans change and a second preceptor joins the rotation, that person needs a sheet of their own with their own exact figure.

What is the 16-hour rule for preceptors not on a site sheet?

UCCS counts only a limited amount of time with a preceptor who is missing from a processed site sheet: anything beyond 16 hours with that person does not count. If someone new starts supervising you mid-rotation, send a sheet for them promptly so their hours are recognized.
